This is a broad category that captures a range of identities, such as trans men (people assigned female at birth who identify as men), trans women (people assigned male at birth who identify as women), and other people who may identify as somewhere in between or outside of binary notions of gender (e.g., nonbinary, gender nonconforming, genderqueer, genderfluid, agender).
Our previous analyses of whether there were differences between LGBT and non-LGBT adults within each racial group revealed a complex narrative on the role of LGBT status within racial groupings in the U.S. While White and Asian-American LGBT adults consistently fared significantly worse than their non-LGBT counterparts on economic and health indicators, the story was less consistent among other racialized groups.
However, many bisexual individuals refute this definition of bisexuality and assert that “bisexual” refers to being attracted to people of one’s own gender and other genders (inclusive of transgender and nonbinary individuals).
What if these labels don’t really fit me?
- People often do not fit neatly into any one sexual orientation group.
These negative views may contribute to mental health problems.
- Therapists’ ethical obligations related to sexual orientation and gender include:
- Evaluating their own beliefs and feelings regarding sexual orientation and gender, and how these beliefs and feelings impact their work;
- Obtaining the appropriate training, experience, consultation, or supervision necessary to provide competent services;
- Referring clients to appropriate treatment providers in situations where the therapist does not have the needed training or experience to provide competent services.
- It is best to ask clients how they prefer to be referred to, including their name, pronouns, sexual orientation, and gender identity.
- The degree to which a client’s presenting problem is related to sexual orientation or gender identity varies.

Gender includes:
- Gender identity: How someone labels themselves or identifies.
- Gender expression: How someone presents themselves, including their physical appearance and behaviors that express aspects of gender identity or role.
What does it mean to be transgender?
- Someone’s sex assigned at birth may or may not be the same as their gender.
- People whose gender aligns with their sex assigned at birth are referred to as “cisgender.” Most people are cisgender, although many people may be unaware of this term.
- “Transgender” refers to when someone identifies as a gender that is different than their sex assigned at birth.
This occurs because, as al-Gharbi writes, “the widespread notion that people who are most oppressed can understand society most clearly… and should be given more deference and respect.”
It’s a provocative theory, and it may help explain how our current culture has altered social incentives. Fewer White than POC LGBT adults reported experiencing food insecurity, unemployment, use of Medicaid for health insurance, and living in a low-income household.
When someone’s external genitalia do not fit with expectations for males or females (referred to as “intersex”), other indicators (e.g., internal genitalia, chromosomes, hormones) are often considered in order to assign a sex.
“Gender” refers to a person’s internal sense of being a man, a woman, or another gender, as well as to gender experiences that are somewhere in betweenor outside of binary notions of gender (e.g., nonbinary, gender nonconforming, genderqueer, genderfluid, agender).
Finally, AIAN and Latinx LGBT adults have among the highest rates of being uninsured for health care (see Table A2).

Considering Social Context when Examining Race and SOGI Disparities
As we have noted across the reports in these series, there are several core demographic and social life differences between LGBT and non-LGBT people that may be factors in the health and economic differences observed.
However, emerging research focuses on applying CBT techniques to therapy with gender minority clients. A decade earlier, young women were only slightly more likely to identify as LGBTQ than young men. There are a few explanations worth considering.
Is it Rising Social Acceptance?
For a very long time, many gay or lesbian people felt compelled to hide who they were to avoid public condemnation, social stigma and even violence.
For instance, in 2015, 10 percent of young women and six percent of young men identified as LGBTQ. As such, the collection of reports in the series highlight evidence that White male heterosexual cisgender status remains a salient nexus of privilege.
Yet, the findings overall do not implicate a simplistic narrative of the impact of an intersectional marginalized social status on well-being.

The overwhelming majority of conservative, moderate and liberal seniors are straight.
It would be a mistake to assume that any current trend in sexual identity or preference is destined to continue. m. The percentage of Americans who identified as gay, lesbian, bisexual, transgender, or queer was relatively low and inching up slowly year over year.
Importantly, politics appear to be a much stronger predictor of LGBTQ identity among young adults—very few older Americans across the political spectrum identify as LGTBQ. For example, someone might usually only experience attraction to people of a different gender, but on occasion experience attraction to someone of the same gender.
The previously published reports for this series provided data on the well-being of LGBT adults by racial groups in the U.S., with attention to areas in which sexual orientation and gender identity (SOGI) disparities may exist. Human sexuality as it is manifested in modern society is not necessarily an immutable state of being or preference.
